If your server is still including version 4.59.2 of Plex Web, you might want to update your server before further troubleshooting things. That version shipped with PMS releases in July/August 2021 (PMS 1.23.5 to 1.24.1).
Beyond that, common issues can be:
- very bad file naming / folder organization
- permission issues
- weird unsupported file formats
- files are actually scanned but mismatched as something entirely different (e.g. due to file naming or conflicting embedded metadata)
