I'm having a problem with the Xbox One app, it keeps showing "Unexpected Error" no matter what I do or try.
The app had been working perfectly fine for months (since it was released until 3 days ago), my Xbox has a solid (wired) network connection, Netflix works fine, Store, multiplayer, etc etc works perfectly fine. If it makes any difference, Network tests show NAT Open and no multiplayer problems.
When I launch the Xbox app I can briefly see the name of my remote server, and after a few seconds I get "Unexpected Error" and a message saying I should make sure I'm connected to the network.
I've tried a hard reboot, I've deleted and reinstalled the app multiple times, I've signed out and back in a bunch more times with PIN and with password; nothing works!
I can use my phone, tablet and computer from the same network without any problems, it's just the Xbox app, which coincidentally, is the one I use most of the time.
My server has a Lifetime Plex Pass and the account I'm using on the Xbox does as well. I tried logging in with the server owner account and I get the same result.

Is there anything I can do on my end to make it work again?
I am in the same exact position that you are in. The Unexpected Error Please check your device is connected to your network has been randomly popping up for me. I can play a movie for about 5-10 minutes and then I get the error. I go to the retry option and it works again for about 5 more minutes and then repeats.
I first thought it was a problem with my network but it seems that only Plex on the Xbox One is doing this. I have not had any problems with Plex on my smart TV nor through a web browser. This started a couple days ago for me as well.
I am currently doing other networks/computer tests to determine if there is something that would be causing the XBone Plex app to behave like this. I will report any findings I happen to stumble upon.
Update#1
I ran some ping tests last night and got some random packet loss (1%) from my media server. Again I am not having any problems with other Plex client/players throughout the house. I would think that the packet loss would be negligible.
Update#2 (resolution)
Plex is now working again. My problem was that my Plex Server (which was using wireless to connect to my network) was disconnecting due to a wireless problem I was having. This resulted in the Xbox One Plex client to lose connection. This is a bit odd since nothing else had issues. I am assuming that the code in the Xbox One client doesn't allow for much packet loss to the server, I don't know.
I ended up connecting my Plex Media Server to my network via an ethernet cable and my problem was solved. I have been testing this for a couple days now with no issues. If there is a problem with your network the Xbox One Plex app may not like it and give the disconnected error. Maybe the Plex app needs more fault tolerance built in.
