Well, my issue is back 
I just upgraded to the latest version as well of 1.15.3.876. Also I saw in the logs that the quality is 95, and signal is 100 now. so it’s not an issue with my signal. Also I can view the channel just fine outside of plex… Roku is also unable to view the channel too. But I am primarily trying to access this channel using chrome.
Attached are my logs, and now I am seeing a good signal for channel 749, but I cannot tune it. It comes up with error 16 (Recording failed. Please check your tuner or antenna.) Attached are latest logs, and I captured them right after the 2nd attempt to tune the channel.
