Duplicate movies showing as new items

For the past few months, when I add a movie that already exists in my library, it is being added as a new, separate movie in a second location of the same name as my 1st location. Previously when doing something like this, it would be added to my library but would appear under the title that was first added to Plex. Then I would get an option to “Play Version” and I could select from either of the two files.

Now I have to manually search for the movie and select it to be presented the following screen where I have to make a selection from one of two locations that have the same name.

This is not expected behavior and incredibly annoying. Any ideas on why this would be happening and how to fix it?

Perform a “Refresh Metadata” for the whole library overnight.

Did you recently switch the default metadata agent of the library?

I did change the meta data agent but only because this issue was happening, and that was just recently.

I will attempt a refresh of the meta data tonight.
