When I look at Live TV guide data, I see episode names and descriptions just fine. Shows are tagged as “New” and show the proper season and episode numbers. If I record a show, however, it shows the season and episode numbers, but there is no episode name or description whatsoever.
I just created a new TV library to record these shows into, and deleted the DVR itself from Plex and re-added it (although all of my previously scheduled recordings / series were still there). When I created the new library, I used all of the default options. I have deleted and set up series recordings again, as well as recorded shows ad-hoc, and the result is the same - no name or description. In the past, the library I used (which was a dedicated DVR-only library as well) was able to get episode info. Now neither library does - actually, it seems the new library gets episode info sometimes, though not all the time (even on repeat airings). I don’t know when this first showed up.
How can I get the proper episode names and descriptions to show up?

I seem to have a similar situation to yours. The dedicated disk for my recordings failed. I replaced the disk and created a new “recorded tv” library. I was able to salvage some recordings from the failed disk and transfer them to the new disk. When I look at these in the plex interface, I can see all the expected metadata.
However, new recordings made since I replaced the disk have no metadata. Like you, I see all the relevant info in the channel guide but not attached to recordings.
Perhaps unrelated but I just noticed that one of the half-hour shows I recorded as a test uses about 350 MB on disk, but Plex reports the playing time as 16 hrs 58 min. The actual duration is about 20 minutes, so 10 min too short. Others however report a more realistic duration. Maybe just an isolated glitch on that one show.
I spot checked a few of my recordings and the file size and episode duration all line up.
I have in the past seen the issue you experience, and the resolution for me was to reposition my antenna and move the Plex cache directory off my NAS (see my question here; I don’t think I mentioned the insanely long episode durations reported by Plex in that question, but I haven’t seen that issue since)
I have noticed this strange anomaly myself with my recordings. But in some cases (that is when recording from some networks), the meta data is filled in properly. However, in most cases, all I get is “Episode x - Episode x” as the name of the episode and no description. I can manually do a “Fix Match” on the series which then fetches the correct metadata as well as the correct posters for the series and season. So, when first recorded, in some cases, the posters are not what I would expect, and no metadata. When I Fix Match, the posters are correct and the metadata is filled in. So, is this a bug, or a configuration issue?
I am experiencing the exact same behavior, no episode metadata but when I fix match on the show, it works.
To expand on this, the issue is only with the first recording. So if I set a show to record all episodes, episode 1 will have no metadata and episode 2 will pull down the metadata fine, so it’s not an issue with the show matching.
If I then delete those recordings after watching them, the next one that is recorded will not pull the metadata, but the subsequent episode will.
