Fixing indirect connect

hi everyone - hoping can steer me in the right direction.
I am currently running my server on my Synology DS216+II NAS and using a Synology RT6600ax router.
When I am away from my house, and try to log onto a Plex app off a Smart TV, it tells me that I am using an indirect connection.
I have tried to follow some suggestions regarding port forwarding, but nothing seems to work.

any help would be greatly appreciated!
