When configuring multiple HD Homerun tuners, any tuner configured after the first inherits the channel maps of the first tuner. This is a problem if configuring cable and then an off-air tuner, or vise versa.
The Plex DVR supports either Cable or OTA. tuners and will configure the channels based on the first tuner added as you have discovered. See the DVR & Live TV Frequently Asked Questions article:
Multiple tuners can be added to your server from within the Plex Web App. Tuners should be of the same type; all OTA or all cable, on the same Plex Media Server

Thank you for this tip, being unable to load the correct channel mappings and subsequent Guide Listings for both OTA and Cable Card Tuners was very frustrating!
