I’ve been a plex pass subscriber since I first started using Plex.
My friend has been able to access it fine for over a year, with no problems. (From iOS and AppleTV). Just recently I replaced my router, and it stopped working. I figured out that I needed to re-setup the port forwarding. It now works for them on AppleTV but they are now getting “Playback limited” after 1 minute of playback on iOS. Is there something else I need to change to make this work? (Why might it have stopped working?)
That error message means the app isn’t paid for, which is not related to your Plex Server or your network configuration. Is your friend a member of your Plex Home, or does your friend have his/her own Plex account and you added them to your server as a friend?
They have their own Plex account, and it was working for him until I changed out my router. Has something changed that would have caused a change in behaviour? Perhaps causing the connectivity disruption to have trigger some sort of license renegotiation?
Maybe it was logged in with your account by mistake? Then after you got the connection fixed he had to log in again.
Has your friend paid for the app?
If they paid for it and can prove it, contact Plex.

Nope. Your friend purchased the app, which is what unlocked it and allowed him to watch more than one minute of a video. That has nothing to do with your server.
