This has happened to me recently when I changed my network.
The short version is that under settings, Plex says the server is fully accessible with a green sign outside of my network. However, my iphone app cannot connect and says direct connection is not available.
Interestingly, I can log into my server outside my network via any web browser and it works fine!
I have signed out and deleted the IOS app and reinstalled but no joy. iPhone cannot connect to the server regardless of my using wifi outside of my home or use cellular network.
Any insight?
