FYI - so far I can’t replicate this in Kodi, which I finally installed due to annoyance. I’ll continue to watch the movie I can replicate this in in Plex.

Audio is really out of sync. Very annoying ! Any chance to have a fix soon ?
I have been having issues with Audio / Video lip sync issues for months now on my Nvidia Shield and it’s not just on Plex. I’m having issues with the HDhomerun App and some streaming apps like Disney+ also. However, Some apps like Netflix and YouTube run fine! It’s driving me nuts. The audio sync features on my LG OLED Tv, Vizio sound bar, or 2017 Shield do not seem to fix the problem.
Bumping again, I am having issues on my 2017 shield. I will need to use Kodi for the interim with PlexConnect. A bit frustrating that this has gone on for so long. Disabling audio passthrough resolves it but then I am limited to just 2.0 stereo and the sound is garbage.
I am having the same issue, seems to go away when I enable the old player.
Nevermind, resurfaced again with the old player.
Sound regularly goes out of sync. Using HDMI passthrough.
Don’t have these issues at all when using kodi on the same device
Using Plex on a windows 8.1 system. Plex client on the Shield was having audio/video sync issues. After a few seconds of playing a video it seemed the video “hicupped” and then the sync error.
I disabled Dolby Audio Processing on the Shield. Sync issues are completely gone.
Not sure if this has been mentioned. I didn’t read all the replies here. Hope it helps somebody.

doing the above solved some issues for me at first but it then resurfaced.
Still no fix, no ETA on a fix. 2 months on and the usual tumbleweed from Plex. Ditched it off now for Emby (free too…) and working perfectly.
i am new to the Shield and Plex. have a 2019 Shield Pro running Plex .
have noticed some minor skips in playback occasionally, where it appears like a dropped frame or two.
last night was the first time i saw drastically out of sync audio (perhaps 24 frames out), playing back UHD HDR with Dolby True HD 7.1 audio.
i would get hard frame skips - very noticeable - occasionally punctuated by a frame or two of black.
and , by 30 minutes in, very out of sync audio.
stopping and restarting playback did not help.
plex is set to direct play / no transcode and Shield is connected to Denon X3500, with denon connected to LG OLED.
prior to this UHD title, i have seen very few issues. most of my content is compressed 1080p with AAC audio.
I have come to the conclusion that plex is absolutely useless on the shield when you are playing back files with HD audio and outputting to an amplifier, as everything is out of sync.
My recommendation: Stick with kodi and use the plex add-on, way more reliable, or look into alternatives like emby, as all plex seem to care about is adding PVR functions and don’t care that the actual core component, high definition media AND audio playback, is completely broken on a device that is sold as a plex server.
Can we get a bit more information?
Namely:
Setup/Hardware in use, and how it’s connected?
What kind of content is being played? (Can we get a sample?)
Does it happen when resolution / refresh rate switching is off?
Is is transcoding?
- Nvidia Shield TV Pro 2019 connected to a yamaha rx-v385 amplifier over hdmi, which is then connected to a LG 86NANO91, all over hdmi 2.1 cables
- Any 4k mkv video with a dolby truehd audio track.
- Resolution switching is off however I havent tried it with framerate switching turned off. Kodi has framerate switching on and exhibits zero issues of the audio sync issues that plex has.
- Direct play
Same issue but it is only limited to the NEW Player. It is flawless on old player
Can you try it with framerate / refresh rate switching off and see if it still occurs?
I have a comparable setup (Shield Pro 2019 -> TrueHD capable soundbar -> Sony Z9D), and I haven’t been able to reproduce this

Just tried on the same video file with frame rate switching turned off and the sound seems to be in sync.
Seems frame rate switching was the culprit for my setup. Is this a known issue and is there any potential fix planned? Note that kodi also has frame rate switching enabled and seems to have no sound sync issues.

No, not a known issue, but it was a data point we wanted to check. Something we definitely want to fix! 
Can you reproduce, and grab some logs, and a sample file for us, please?
