There are several missing channels from the Las Vegas, NV - 89128 programming guide
The lineup name (you’ll find the lineup name on the Live TV & DVR settings page under CHANNEL SOURCES ) - see screenshot below

Thank you again for submitting these. How do I know if and when the channels are added? Do I just refresh the guide and then go into add the channels or do I have to delete the HD home run and then re-add it?
In my experience, you wont get a notification that they were added. Wait a few days, then try adding the channels. You do not need to delete your HDHomeRun setup.
When they agree to add channels - it usually happens within a couple of weeks, When they deem channels from specific stations are out of scope, then it may take many months for them to consider if they would get added
In the past they have indicated that low power stations are out of scope for inclusion in their database and they would consider each on a case by case basis
For the request for your channels, I have received an initial response that they will add the WQAP channels and will look into if any others can be added.
So the ones they agreed to add are:

and I do now see them in the lineup for your zip code
To pick new channels in the lineup, you would need to go in Plex Web to
Server Settings → Live TV & DVR → DVR Settings
Click on the x enabled link for the DVR
Scroll down to the tuner channels that are not mapped
Enable and then check the drop-down in the EPG channels on the right side if there is a channel in the EPG for this DVR tuner channel - if not, untick the tuner channel selection box
So you can map 36.1 to 36.6 this way and then save
I am sorry but I have no way of starting an investigation without a country/post code/lineup name - I have no mechanisms for looking up channels by channel name/country - the starting point for any investigation is country, zip/postal code, lineup/guide name
This has been referred to Gracenote. Note that getting logos appears to always take a long time to be done
In future - when specifying channels here - please also include the channel number in the lineup - that would speed up identifying the channel when looking into issues - in your case - it was channel 95.
Also it would help if you have any web page links relating to the channel or lineup in question
