Ultimately there was something odd going on with the DVR Tuners in Plex. I noticed that live TV tuning would take place only on Tuner2. Once two channels were tuned, I noticed that I could occasionally tune a third item but it was only on a specific channel (Tuner1). Both tuners had the exact same EPG channel list loaded too. I finally decided that I was going to remove my tuners from Plex and start over. When I removed the working tuner (Tuner2) my guide for Tuner1 suddenly reflected only 3 channels available (even though around 50 were selected as enabled in the EPG). When I deleted Tuner1 and started over, I was able to get a fresh EPG channel list (about 110 channels vs. nearly 500 repeated items) and all 4 working tuners.
Clearly something got mangled over the years of service and needed a purge. I wanted to add this follow up in case anyone ends up with a similar issue.
