Now, regarding your MP4/M4V problem (file name, go figure) - that you may not have even known you have - Plex will stubbornly read and react badly to Title Fields containing those Horrific Scene Names inside MP4/M4V files. You can go through all 17 million movies/episodes and remove those Bogus, Hideous, Embedded Titles - or:

You can lower the priority for Local Media Assets to the bottom of the Active Agents under all tabs in TV Shows and Movies (Drag/Drop):

Green: Where it should be

Then, if Refresh Metadata doesn’t do anything - The Plex Dance will (should - providing you named the dam thing correctly):
