You’re using at least some MP4 files. If there are embedded Title Fields within containing strange/bogus info, that’ll bork things quite effectively as Plex will prefer embedded Title Fields in MP4/M4V files over pristine file names - unless:
Drag and Drop Local Media Assets to the bottom of the Active Agents under ALL tabs in TV Shows and Movies:
Red: Where it was
Green: Where it should be
Refresh Metadata and see what happens. If nothing happens Plex Dance and see what happens:
