Movie file missing in library, but it's in the folder and it plays directly. How do I find out why?

Strictly follow Plex naming and organization requirements, adding the IMDB/TMDB ID if needed. If you’ve extra info in the file name, place it in square brackets so Plex will ignore it.

Then Plex Dance the movie. Do not skip any steps. This removes any incorrect matching information from the Plex database.

Example: /Desert Command (1946) [video/audio/etc info].mkv
Example: /Desert Command (1946) {tmdb-137235} [video/audio/etc info].mkv

Note: If the movie is in the MP4 container, poor embedded metadata can cause Plex to mis-match the file.

Either (a) edit/delete the metadata or (b) tell Plex to ignore the metadata: Edit the Library → Advanced → uncheck “Prefer local metadata.”
