Movies not appearing with correct title all of the sudden

All of the sudden around 1/2 of my movies are not being added to Plex with the correct title. I have never had this problem, it’s just started in the last week or so. If I add 10 titles, 5 of them will still have the file name in the title instead of the metadata from the detected movie. The images and descriptions are right, but the movie title is always wrong.

1 - scanning A.Streetcar.Named.Desire.1951.1080p/A.Streetcar.Named.Desire.1951.1080p.mkz
2 - movie will be detected correctly, updating all metadata correctly
3 - Movie title will be A.Streetcar.Named.Desire.1951.1080p and I have to manually go update both the title and the sort title and save

Any ideas what is causing this to happen all the sudden? I can unmatch the movie and match with a different source and it will do the same exact thing.
