There’s Plex Media Server - #666 by gbooker02 v. 1.41.5.9522 available now. Is all this behind us? Anyone? @Atomatth ? Bueller?
I still have an open issue linked to this thread. So I’d say, no. I have been able to reproduce the issue.

This usually results in a Weak Signal error message
It’s looking like the resolution is going to be a bit involved and requiring major updates on PMS. Thankfully this work was already scheduled but unfortunately will take some time.
My understanding of the issue is that it’s limited to Windows hosts with older CPU/iGPU. So far Roku is the client where this has been confirmed. I have seen others chime in mentioning other clients but without logs
.
So for anyone tracking this thread or jumping in late, if you think this issue matches what you’re seeing please share your PMS logs. Please also call out any affected clients.

Thank you for your reply @Atomatth … as an update, I still very occassionally get the playback error. However, it has become few and far between, so I feel it’s merely an annoyance. No big deal. Luckily, I haven’t seen the issue while recording, only while watching live.
For now I can live with it. But I’ll also keep an eye here & look forward to the resolution.

I last posted on November of 2024. Recently, I clicked on the “Download Updates Screen” in Settings/General, PMS was upgraded to v.1.42.1.10060-4e8b05daf of August 11, 2025.
I am running Windows 11, with all current updates.
After the update to v.1.42.1.10060-4e8b05daf my Recorded TV Shows began to not record the show completely. Some partially (About 1/2 the show) and others only the first 4 to 10 seconds or so. I have uploaded the the Logs from Troubleshooting for review.
I thought I would wait to see if the issue was resolved.
Please let me know if other information is required.
It’s been a while since I posted on this thread but live TV is still not working reliably for me. In my case, that’s true for IOS (iPad) or Android. The error messages vary depending on whether I have HW transcoding enabled. When I turned off HW transcoding, live TV played for a few minutes then stopped with a “The requested URL was not found on this server” message. With HW encoding enabled, I get a long error after a few minutes that ends with “unable to get playlist before long download timer.”

Interesting update. The stream that fails is the 1080 resolutions transcoded via HW.
In the screenshot above, the two lower resolution streams, (480i and 720p) have been streaming fine for a half hour+. The 1080i stream going to iOS will only go for a couple minutes before returning the error “The operation couldn’t be completed (CoreMediaErrorDomain error-16839 - Unable to get playlist before long download timer.)
Update 2: It’s not the resolution. A 480i stream on iOS just failed with the same error. Now it appears to be an iOS client issue when HW transcoding is enabled.
Your system is pretty FUBAR’d.
Ran Plex as a Service but you probably used different users mixing permissions.
Database is having problems. (GitHub - ChuckPa/DBRepair: Database repair utility for Plex Media Server databases) may fix the physical but if their are logical problems then restore a backup.
Improper values in “Allowed Networks”
Probably have a codecs problems.
Might want to ditch the pseudo HDHomerun tuners.

I see others are still having this issue, as I still am. I have major problems with just one channel, which I think is the tower. I do have fewer frequent issues on many other channels, though. So I think whatever is happening is still a ‘bug.’ @pl_5309 - what do you mean by pseudo HDHomerun tuners?
