Remote access failures have not been a definitive symptom.
Seeing the entry ‘404’ in the server logs at startup with the message (thread title) is definitive .

When I press the login button in Chrome it will not show the login dialog box in plex.tv domain. It works in firefox (cleaned cache - restarted everything)

You have several RAMPED.
I removed the instances of RAMPED which were over 2 years old and not seen since then.
They also had no shared users
I then reset the cert for the current one (seen 45 minutes ago)
You may restart now
