There’s a show that Plex pulled the wrong data from (the name is the same). I’ve already used Fix Match to point to the correct show. However, every time the library resyncs, the wrong metadata for that show reverts back.
However, Plex is pulling the metadata for a highly NSFW anime that isn’t even on theTVDB (so I can’t find the ID).

I’ve uploaded the media info XML files for each of the 3 episodes I have of this show (the thumb for the third episode is the only correct one, the first 2 are from the wrong show).
Looking at your xml your library is using the new Plex TV Series agent. Those other things you described are old legacy agent settings that have no bearing on new agent. All the new agent settings are in the advanced settings for the library or the individual show.
The issue which I think is likely the cause of problems is that you appear to have your library set to just look at your downloads folder D:\Downloads\ then just individual episodes in their own folder. This is very bad for how the agents work. The agents expect each show as a whole to have its own folder and since they don’t every time it scans it is getting confused. The main folder should not be one that is always changing like a downloads folder.
It should be like this where D:\TV is the folder added to library. The {tvdb-363024} at the end of show folder is the TVDB ID that the new agent can use to help match the show
