This was an issue with NTT, a Comcast peer. The traffic was blackholed when the route became unknown.

Can this happen again?
Yes it can. Look at the BGP blackholing of FaceBook (Meta) DNS servers. Plex was not in control of this as it was occurring on the internet backbone.
