"Scan Library Files" function finds files, adds them to library, then immediately removes them from

I have a file folder with MKVs in it of a TV show, properly named in a properly named directory. I know that it’s all properly named because when I point “Scan Library Files” to it, Plex finds the files and puts them in the library, with correct poster, episode names, and so on. In fact, I can navigate to the show, season, and select an episode, and start watching.

But before the scan is done, Plex removes the same show from the library with no warning. If I’ve started watching an episode it just stops and closes the player, returning me to a library that now doesn’t have the show it just added.

If I just keep the library open, I can watch the show appear in the library in the right spot, then a moment later a trash can appears on it and it disappears.

I have full access to the episodes on the disk (and have tested the files by playing them in a media player.)

If it just wasn’t finding it, I would keep playing with things like file names and so on. But the Library Scan is finding it…adding it - completely properly! …then instantly removing it.

I looked in the log and couldn’t find anything of interest. “Empty trash automatically after scan” is not activated.

I’m befuddled. Any assistance is appreciated.

Instead of watching, go to the top level preplay of that show and “edit” the show.
On the “Advanced” tab, verify that there’s not a policy set to delete older or watched episodes.

If that’s not it, here’s another test:
Set your TV shows library to “Episodes” viewing mode.
Then enable the “Duplicates” filter.
If you get any, inspect the Plex media info of these. Chances are that your freshly added files are being attributed to a different show, after the initial scan has finished.
