Generally a promblem, bought PLEX PASS - -for XONE and X360, do not see a computer with PLEX. Through the media server all the rules.With Android app on Plex comes sees and plays. What could it be? Reinstalled win came with 8 - -Just Trouble.

Do you have the most recent version? You never state the version of plex you have in your post. Current is 0.9.11.1
I have this same problem. I'm using 0.9.11.1, and it used to work fine, but then one day I tried to connect whilst my server PC was connected to my work VPN and then boom, hasn't been able to connect since. I've tried everthing, re-installing app, quitting app, power cycling xbox, the whole gamut. I paid a lot of money for plex pass and now I can't use it, I think I might have to just get a refund on the plex pass if I can't find a fix soon.
Your server has to be connected to the plex website for the Xbox app to work. Check under settings/server/connection.
I'm using 0.9.11.1.678
Even with Versionsnummer 0.9.11.1 the Server has to be connected to the Plex Website 
Just Check the Setting slydog75 posted.
And/or login under Plex.tv and habe a Look under the Server Tab
