I take it then, that the internal IP is set to the IP you’ve reserved for the Synology NAS within your home network (not the synology.me URL) and the port 32400. Are you then also able to connect to your PMS from a Plex client outside your network? (e.g. disabling WIFI on your phone and either connecting via http://app.plex.tv/desktop or the native Plex client on that phone)?
