I am constantly having this issue when I access live tv. It happens when I access via Apple TV and my Roku. I make sure that all have the latest version (server, Apple TV app and Roku app). I select a program from the guide, it starts to load but never does, then server crashes.
This is getting super frustrating. What magical settings are there that prevent it from happening?

There is a crash in the intel graphics GPU driver - in this dll mfx_mft_h264ve_32.dll
Can you tell me if you run the server as a service on windows or as scheduled task ? or through RDP ?
If that is not the case, then other area to explore is the version of the driver
There were bugs leading to crashes and this has just been fixed for one the intel GPUs - you could check what version you have and if there is an equivalent version for your GPU - see this post Hardware transcoding (QuickSync) causing server crash - #75 by sa2000
That is what this crash is associated with - it is because there is no desktop session
As from version 1.18.4.2164 of Plex Media Server, there is a new Advanced Setting for the Transcoder that allows users to disable Hardware Encoding whilst leaving Hardware Acceleration enabled and used for decoding. This hopefully would avoid the crashes and would be a better option than disabling all hardware acceleration
