SO many albums have no art showing.
I have ‘Prefer local metadata’ chosen.
I have tried all three options for my chosenLibrary>Manage Library>Edit>Advanced>Album Art.
I really hate the look. Anyone know if I’m neglecting something?
I am also experiencing this issue - if you force “unmatch” on the item does your embedded meta data artwork return?

What a ridiculous PITA. I have several hundred needing this fix.
Subsonic doesn’t do this. Emby doesn’t do this.
Plex is sure making it hard for me to buy in.
Expect you will also find, adding to your frustration, that following a library scan the erroneous matches will pop up again, forcing one again to manually unmatch. This thread suggests that once you force an unmatch it should stay and not rematch erroneously. Un-matching Mismatched Items - Album Art - Must this be done on an item by item basis? - #11 by Fletchesus. But I am finding I need to unmatch daily (following each daily library scan), which is obviously not ideal. Interestingly - I am finding this issue with PMS running on Windows 10 only - PMS on Mac seems to retain the unmatched artwork.
Much thanks for your input. Yep. Following a re-scan it’s a mess again. But not totally! A few of them maintained - just a few. Between this sad state and the issue I and others are having with Artists I can not see me moving forward with Plex. I trialed PlexPass so as to have an equalizer in the iOS app, the otherwise excellent Plexamp. I was very hopeful. But I need to navigate my library! I can forgo the EQ and save money with Emby.
Continuing major frustration with this issue. Anyone seeing any improvement. None here. Thanks.
Possible fix - ensure under Settings > Scheduled Tasks > UNCHECK “Refresh local metadata every three days” and “Refresh library metadata periodically”. So far, my forced unchecked items with custom artwork have been retained following library scan. The other settings are: Scanner - Plex Music; Agent Plex Music; Prefer local metadata checked; Album Art “Local Files Only”. For Agents, “Local Media Assets” is top item for all categories. Possible fix, only - will keep this thread apprised if this solution fails.
