OMG, I just about pulled out all my hair trying to figure this out on my Synology server. I had the exact same version as you did, complete with the exact same symptoms. When I went to my Plex, I didn’t see my server listed at all, and every time I clicked the “Add your media to Plex” icon (the plus sign), it would just go to a download screen – just like you.
Fwiw, I’m running on a Synology NAS. I got this idea from this thread, btw:

I figured maybe something got messed up in the Plex password change a while back, and maybe I needed to uninstall and reinstall with the “Claim Ticket” method to re-claim my Plex server.
Here’s what I did that worked:
- Went to Package Center and clicked the drop-down entry to Uninstall Plex.
- It gave me a few options. I selected “Sign out and unclaim this server. Keep all existing files for future re-installation.”
- Once uninstalled, I clicked on the Manual Install button in Package Center, as normal, to re-install the 1.29 package that we are very familiar with.
- During installation, it asked me if I wanted to Claim the Server (something like that) and offered a link to get a Claim Ticket.
- I went to the linked site and got a Claim Ticket (random alphanumeric value), pasted it in the textbox, and voila, it told me my user had successfully claimed the server!
- FINALLY, when going to Plex now, my user can see all the stuff we used to see (my NAS / Plex Server, media, etc).
Sure hope it helps!
