Try rescanning the library from the list on the left hand side of Plex. If you hover over the server name you will see the three dots appear and in that menu lies the ability to rescan the server.

I believe this is what you are talking about. [Options] These are the options that come up when I click the three dots by the server. I did “scan library files” but nothing changed.

I will give it a try and see what happens.

Try doing a Plex Dance and see if that alleviates it. A lot of these issues can be solved that way. I would do it for the whole library though not just individual files.
