Also noticed, and could be it, that you are using a bad naming!
From the logs:

Should be:
/volume2/Videos/TV/NCIS/Season 12/NCIS S12E16 Blast From the Past .mkv
Note SE12EP16 which is bad, and correct is: S12E16
