Scan Library not picking up all of my media

I have cleared cache, refreshed meta, renamed files all in an attempt to have the plex server recognize new media. But nothing works.

All while Plex was watching…

Renamed them to what?.. Show us your file names and structures:

TV Shows?

With LMA in priority, Embedded Titles within MP4/M4V files are preferred - even over a perfect file name. Local Media Assets should - AT ALL TIMES - remain enabled (unless specifically instructed to turn it off). Typically, you name and structure files BEFORE they go in a library, but many times when you ‘fix files’ you’ll need to Plex Dance to clear out the bundle and create a new one:
