So I called my ISP (Unitymedia/Vodafone Germany) yesterday and told them what we discovered. And by them I mean three different people. The whole story. None of them seemed to get what I was talking about. One of them just replied “So you can’t receive E-Mails?” WHAT?! 
The third person passed my topic to her supervisor which called me today. He knew what I meant and passed the information to the technicians, they will call me back as soon as they figured out what is going wrong here.

Very simple test: traceroute (linux and mac) or tracert to www.plex.tv and plex.tv
set max hops = 100.
It should complete in about 10-12 hops and have IP addresses most of the way.
Failure is going to the end without resolving. (which his Synology does)

I have the same Problem. Upgrade to Cable Max (Unitymedia/Vodafone) with a FritzBox 6591. I also live near Heilbronn in southern Germany.

I have deleted all folder from my sonology und did a clean Install, but it didnt work.
So me and @bwestpha were debugging this weird behavior a bit.
We can confirm that Plex Media Server on Windows (pass version, fresh install) on the same network does not pull any metadata or atleast takes forever…
After enabling the Wireguard VPN for the Windows Machine it pulls every metadata in less than 20 seconds.
We did the same on the former Synology/Xpenology System which is now unraid and it pulls the metadata of 300+ Elements in around about 30 minutes.

To confirm: Once you’re free of their routing via the VPN, everything is back to normal ?
@ChuckPa to be honest. We are no network specialists - just some IT enthusiasts :-/.
What we can confirm is, that it works via a wireguard VPN (the VPS to this is located in Frankfurt near the DE-CIX) and it doesn’t work without it.
I can not clearly say this is a vodafone only problem, maybe there is something between vodafone and cloudflare as well - really - I have no clue.
But at least now I’m pretty sure it is nothing on my side (router, host, etc).
If I were a better network debugger, maybe we could get more information out if this - but this is far beyond setting up a wireguard VPN and testing :S

Having put all traffic in the VPN proves it’s the ISP. You’ve tunneled all traffic to a location outside their influence before setting it free on the open internet . Doing this and all is fine.
A few challenge points you may wish to ask
I prefer to assume this is accidental rather than deliberate but , what if a deliberate action?
Well, I don’t have any problems with any other service and I wouldn’t know that Vodafone has anything against using Plex.
By the way, a friend of mine has DSL via Telekom and with him the search process takes a long time. But the symptoms are a bit different so I don’t want to mix it up here.
