A few days ago, I added a little-known TV show that Plex couldn’t identify. I manually entered the title, cover image, and metadata.
Today, I noticed that Plex has completely messed up several previously well-organized series. Entire seasons and individual episodes from different series have been randomly assigned to the show I had created.
I deleted that series, but the issue persists: all those seasons and episodes are still assigned to it, even though it no longer exists in my library. I can’t reassign them correctly.
How can I fix this issue and prevent it from happening again?

ps. : You can see in this screenshot that several episodes are mistakenly assigned the image of this ghost series (“Le choix des armes”), and they are no longer associated with the correct series.
Did you click on the triple dot and click Fix Match? Also since it is a TV series is your library pulling from “TheTVDB” or “The Movie Database” (Settings → Manage, Libraries → Edit Library → Advanced → Episode Ordering? I figured out mine was defaulting to “The Movie Database” and that is very wrong with many shows. Switching that in the manage libraries helped correct many mismatches for me with episodes.
By investigating further, I see, for example, that only season 4 of Starsky & Hutch is indexed in Plex. Yet, they were all there before, and I haven’t touched these files in years. I realized this when I searched for the series Le Choix des Armes, and an episode of Starsky & Hutch appeared in the search results, but with the cover of Le Choix des Armes.
The same problem applies to more than 80 series, some of which have seen their seasons disappear.
I continued my tests, here is a clear description of the issue encountered.
Incorrect Search Results:
When searching for “Le Choix des armes” in Plex, I filter the results to show only episodes.
Plex returns 97 results, none of which are actually titled “Le Choix des armes”. However, all of them display the custom artwork for “Le Choix des armes”, which seems incorrect.
Episode Details Mismatch:
Clicking on one of the episodes, I find that:
The description corresponds to the show “Rookie Blue”.
The episode is titled “Alerte Enlèvement”.
However, when I play the episode, I realize it actually belongs to the show “Papa Schultz”.
Despite being correctly placed in the “Papa Schultz” folder, this episode does not appear in the “Papa Schultz” series in Plex, as if Plex is not recognizing it correctly.
Have you tried deleting (first back up) your meta data on the server? Also on the media have you tried editing the meta data on the file itself? All I can think of to try to fix the issue.
This sounds like you only changed the poster, but your files are still “matched” to the wrong series. You must never do that and simply overwrite a poster or a series title. It won’t do any good.
Change the overall top folder of that show to include the ID number from the TMDB. If the show is indeed the one I listed above, then the folder name should be something like 1975-1995 le choix des armes (2025) - {tmdb-287629}
