Plex running on a Synology DS218+ (DSM 7.0.1-42218)

On 11/22 I added Season 1 of American Rust and that found all it’s metadata just fine, since then, I have made no changes to my Plex server or NAS at all.
On 11/23 I added Season 1 of Foundation and this had trouble finding even the show title, I did the plex dance and made sure I had the year after it and then it found the show title, but none of the episode names.
Today 11/25, I added the latest South Park episode, that too cannot find it’s show episode name even though themoviedb has the episode name already. I also have all previous seasons of South Park with the same naming scheme and those are fine.
Something happened from 11/22 to 11/23, and I have no idea what.
My files are named like this. Show name\Season 01\ Show name S01E01.mkv
I’ve tried the plex Dance
I’ve tried changing to the TV Database and refreshing metadata
I’ve tried refreshing metadata many times.

I’ve uploaded a log called Plex Media Scanner Analysis as I see the South Park episode in there, but I’m not sure what I’m looking for. the error I see doesn’t tell me much.
I also uploaded the Plex Media Server log after I had removed the South Park episode and added it back.
