Or inspect them yourself. Take a look at the Plex Media Server.log file and seek for messages about database corrupt or malformed.
If you find these, you may have to repair your database.

Are you habitually refreshing all libraries at once? I see messages about updating a tv show and photos at the same time, which is kinda puzzling.
Are there other (successfully matched) tv shows in this library?
When you this library, on the “Advanced” tab, what is selected as the Agent? (don’t change this, yet!)

Actually I’m not refreshing the libraries manually (only recently and only TV shows and Music).
As for the other matches, this item is the only one bugged.
And BTW, I didn’t mention the important thing. Bofore it goes to such a state, It has Match section, but with the blank value, which can not be corrected. So when I choose the option, then it goes to current state irreversibly. Unfortunately I did not snapshot the previous state.
Hm, try this: go to
Settings - Server - Agents - Shows - TheTVDB
Now grab any line with a checkmark and drag it up- or downwards.
Then reverse this action.
This is just intended to “kick the wheels” of the old agent system.
Have you already tried a complete Plex Dance with this show?
Are your files using the mp4/m4v container format?
Grabbing the lines didn’t help. Plex Dance should clear the action as I remember. But issue re-occurs from time to time, so it really looks like a bug.
